DRS LIBSAFE Request Forms for Logins or Containers

Request a new or updated staff login (or request login deletion)

  1. User or Manager: Contact your DRS Login Liaison to request a new or updated Login.

  2. Liaison: Complete and sign a (PDF), selecting the appropriate role/s and owning repository/ies from the expandable table below.

  3. Liaison: Have your Login Approver sign the completed Login Request Form.

Note: This is an interactive or "fillable" PDF form, but the liaison and approver do both need to sign it electronically or in print prior to submission.

Submit the signed forms to LTS by sending a support request with the uploaded request form. Note, please submit a new ticket for each batch of forms. Please do not add new requests to existing tickets.

Available DRS user roles with functional permissions (click to expand)

Request a new or updated DRS owner repository or container

Existing administrative or billing contact should submit a request to LTS Support with the new owner repository and container information.

Required information:

  • Owning repository name

  • Name and path of container to be created/updated

  • Administrative contact name and Harvard directory email

  • Billing contact name and Harvard directory email

  • 33-digit Chart of Accounts code

  • Brief description
