The submission form is open annually for review. A sample form is provided above to help with preparation. 

 The goal of the form is to give an detailed idea of the project scope and content, but areas that would require in depth conservation concerns can be explained more generally. It is not appropriate to request conservation reviews in advance of filling out the form, as it will be sufficient to give general answers around condition questions.
